Dear Parents/Guardians:

The Student Body Government is proudly sponsoring the theme “VinNeon”, in combination first three letters of Vintage and Neon, prom at Muretta’s well in Livermore. The event will take place Thursday, May 9, 2013 from 6 P.M. to 11 P.M. The ticket will cover dinner, dessert, dancing, entertainment, and souvenirs. Transportation by school bus to Muretta’s Well will be provided with the bus leaving CSD at 4:30 P.M.

Pricing is below:

BEFORE and ON April 9, 2013
$85.00 for SBG Member
$95.00 for Non- SBG Member and Non-CSD student

AFTER April 9, 2013
$95.00 for SBG Member
$105.00 for Non SBG Member and Non- CSD student

DEADLINE: MAY 1st
Enclosed are several forms to fill out and sign. CSD students are allowed to bring only one prom date who is not a CSD student. All guests must be pre-approved by their school administrator and our administrators by May 1st, 2013. The following forms as follows:

• Prom Permission Form (CSD student only)
• Prom Permission Form (Non- CSD guest)
• Transportation Permission Form

Please review all of the forms carefully and sign where indicated.

On Thursday May 9th, your child will be excused from class at 12:25 P.M. so he/she may prepare accordingly and dress up for prom.

IMPORTANT NOTES:

• Any student who chooses to miss a class before 12:25 P.M. on May 9th to prepare for prom will lose his/her privilege to attend the prom. We have experienced problems in the past where students would be “too sick” to go to school, but showed up for prom. These students will not be allowed to attend the prom.
  
  
• Students are not excused from classes before 12:25 P.M. to pick up their dates from the airport or other locations.
  
  
• The prom ticket is NOT refundable. However, for any student who is on Academic Probation after May 1st he/she will not be allowed to attend the prom. If he/she pays the ticket prior to the above dates, the ticket may be refunded fully or partially depends on expense already paid.
  
  
• Students are allowed to rent limousines before the prom only.
  
  
• Students will be allowed and expected to show up at school at 10:00 AM the next day, Friday, May 10th.
  
  
• Non CSD guest are not permitted to sleep in cottage.

Once again, the deadline is May 1st to purchase a ticket and submit the following forms.
