Athletic Forms

If a student wants to come out for one of our sports, he/she must turn in the Athletic Clearance Packet before participating in tryouts, practices, or contests.

The packet includes:

• 2012-2013 Athlete’s Agreement
• 2012-2013 Athlete’s Agreement (Spanish)
• 2012-2013 Athletic Clearance Form
• 2012-2013 Athletic Clearance Form (Spanish)
• 2012-2013 Medical Authorization
• 2012-2013 Medical Authorization (Spanish)
• 2012-2013 Physical Examinations
• 2012-2013 Physical Examinations (Spanish)
